Thank you for your assistance during this unannounced complaint inspection. This on-site visit was conducted on March 10, 2020 from 10:50 a.m. through 1:05 p.m. This inspection was conducted a result of complaints that were received by the Valley Licensing Office on February 6, 10, and 28th of 2020, regarding the allegations of ratios, medication administration and hazardous substances. The center was toured, medication administration logs reviewed and two staff were interviewed. The evidence gathered during this investigation does not support the allegation, therefore, the complaint is not valid.
