Comments:
Thank you for your assistance during this unannounced monitoring inspection. This onsite visit was conducted on March 14, 2022 from 11:00 a.m. through 11:53 p.m. Upon arrival, there were 87 children in the care of 17 staff ranging in ages from 4 months through 5. The medication and medication paperwork was reviewed for two children. The following areas were observed: handwashing/diapering procedures, infant feeding, transition and outside play. The records for 5 children and 15 staff were reviewed remotely.

Based on review of staff records, the center failed to have two staff members obtain a tuberculosis screening within 30 days of the date of employment.
Evidence:
1. Sixteen staff records were reviewed. 2. Documentation reviewed for Staff #2 revealed a tuberculosis screening was done on 02/21/2022. Date of hire was 10/18/2021. 3. Documentation reviewed for Staff #4 revealed a tuberculosis screening was done on 03/14/2022. Date of hire was 11/22/2021. 4. The director confirmed tuberculosis screening for Staff #2 and Staff #4 were not completed within 30 calendar days of the date of employment.
Plan of Correction:
Staff will obtain a tuberculosis test or screening within 30 calendar days of their first date of employment.
