Comments:
An unannounced monitoring visit was conducted on July 21, 2022 from 11:30 a.m. through 12:30 p.m. Upon arrival, there were 74 children, ages 6 months through 12, in the care of 14 staff. No medication was being administered at the time.
Areas observed were: outside play, hand washing/diapering procedures, lunch and nap. Children appeared to be well aware of the daily routine and schedule.

Based on observation, the center failed to obtain the required ratio in one classroom.
Evidence: 1. Nine children ages 8 months to 16 months were observed with 2 staff members from 11:40 a.m. until 11:44 a.m. 2. The ratio for birth to 16 months is one staff or every four children. 3. Staff confirmed they were not in ratio.
