Comments:
An unannounced monitoring inspection was conducted on August 3, 2022 from 11:45 a.m. through 1:00 p.m. This inspection was initiated by a self- report from the center that was received on August 1, 2022 regarding the supervision of children. Video recording was reviewed along with 3 staff and 3 children records.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility. A conference will be scheduled to discuss an Intensive Plan of Correction.

Based on interview, review of documentation and of video recording, the licensee failed to ensure that the center?s activities, services and facilities are maintained in compliance with the policies and procedures that are required by these standards.
Evidence:
1. On August 1, 2022, seven 2-year old children were outside on the playground with 1 staff member supervising. 2. At approximately 10:20 a.m. Staff A was observed walking one child back inside the building to the classroom for a diaper change. 3. Six 2-year old children were observed playing on the playground out of sight and sound supervision of staff at approximately 10:20 a.m. 4. The center?s current policy, as of 8/31/2022, states ?No staff are EVER allowed to leave a child who is 12 and under in a classroom or playground by themselves.? 5. The director confirmed Staff A did not follow the center?s playground policy and procedures

Description:
Based on review of video recording and staff interviews, the center failed to ensure children, under the age of 10, were in actual sight and sound supervision of staff.
Evidence:
1. On August 1, 2022, seven 2-year old children were outside on the playground with 1 staff member supervising. 2. At approximately 10:20 a.m. Staff A was observed walking one child back inside the building to the classroom for a diaper change. 3. Six 2-year old children were observed playing on the playground out of sight and sound supervision of staff from approximately 10:20: a.m. through 10:21 a.m. 4. Staff A was observed walking back to the playground at approximately 10:21 a.m. 5. The director confirmed Staff A walked inside the building with one child and left 6 children out of sight and sound supervision.
