Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on November 1, 2022 from 11:20 a.m. through 1:15 p.m. Upon arrival, there were 82 children in the care of 15 staff. Seven staff and five children records were reviewed and updated. The medication and medication paperwork was reviewed for one child. Areas of non-compliance may be found on the violation notice.

Please complete the plan of correction with 5 business days and return to the licensing inspector.

Standard #: 22.1-289.035-B-4
Description: Based on staff record review, the center failed to obtain an out of state central registry check on two staff members.

Evidence:

1. Seven staff records were reviewed and updated.
2. Staff #5 and Staff #6 did not have documentation of obtaining an out of state central registry check.
3. Staff #5's date of hire was documented 10/24/2022. Staff #6's date of hire was documented 02/29/2021.
4. The director confirmed these checks had not been obtained.

Standard #: 8VAC20-780-510-G
Description: Based on review of medication, the center failed to have one prescription medication labeled with the child's name, the name of the medication, dosage amount, and the time or times to be given.

Evidence:

1. One medication and paperwork was reviewed.
2. One Albuterol Inhaler did not have the pharmacy label attached to include the child's name, the name of the medication, the dosage amount and the time or times to be given.
3. The teacher confirmed the medication did not have the pharmacy label attached.
