Comments:
An unannounced monitoring inspection was conducted on August 14, 2023 from 10:15 a.m. through 12:25 p.m. Upon arrival, there were 76 children, ages 4 months through 9, in the care of 17 staff.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review the center failed to obtain documentation of additional immunizations once every six months for one child under the age of two.
Evidence:
1. Five children records were reviewed. 2. Child #5 did not have documentation of obtaining updated immunizations. 3. Child #5 is 22 months of age. 4. The director confirmed updated immunizations had not been obtained.
Plan of Correction:
The center emailed the parents of the child and notified them about the immunization records. The parents were also notified that the child would be dismissed if the parents did not produce updated records. This was our second and final attempt to notify the parents. Supervisory leads and administrative staff reviewed Virginia licensing standards on record keeping and children's files.

Description:
Based on review of medications, the center failed to notify the parent medication authorization had expired and to pick up the medication within 14 days or dispose of it.
Evidence:
1. Medication and medication paperwork was reviewed for two children. 2. Child #6 had expired medication authorization to administer Tylenol. 3. Medication authorization expired 05/05/2023. 4. The teacher confirmed the medication authorization had expired for Child #6.
Plan of Correction:
The expired medication log and the medication were removed from the premises and returned to the parents. The parents were notified about the length of time medication logs and appropriate storage of medication. Administration also created a document to track all medication logs and medications in the entire center.
