Comments:
An unannounced monitoring inspection was conducted on February 26, 2024, from 11:40 a.m. through 1:20 p.m. Upon arrival, there were 79 children, ages 4 months through 5 in the care of 17 staff.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of 5 children and staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review, one immunization record did not include a signature and date by a physician, his designee or an official of a local health department.
Evidence:

Based on review of medication, the center failed to return medication to the parent or dispose with 14 days of authorization expiring.
Evidence:
1. The medication and medication paperwork was reviewed for Child #7. 2. Parent authorization to administer medication expired on 01.22.2024. 3. Staff confirmed the medication had not been returned to the parent or disposed of within 14 days of the authorization expiration date.
