Technical Assistance:
Required background checks:
- Sworn Statement or Affirmation (required prior to employment and every five years thereafter)
- Fingerprinting conducted through Fieldprint (required prior to employment and every five years therafter)
- Search of Central Registry / CPS Check (required within 30 days of employment and every five years thereafter)
- Out-of-state search of the central registry (required within 45 days of employment if the employee has lived out of state within the past five years)

Comments:
Licensing Inspector arrived at 10:30 and departed at 11:45 AM to conduct a religiously exempt child day center code compliance inspection in conjunction with a subsidy health and safety inspection. At the time of the inspection, there were 10 children (ages 6 weeks - three years) in care with three staff. Children were observed participating in finishing snack (grapes & cheese) and participating in free play.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on review of four staff records, it was determined that the facility did not ensure that that each staff member had a completed search of the central registry on file within 30 days of employment.

Evidence:
None of the staff records reviewed contained a completed search of the central registry.

Plan of Correction: Although submitted, the searches of the central registry have not yet been received. Once received all results will be forwarded to the department and copies maintained in the staff records.

Description: Based on review of four staff records, it was determined that the facility did not ensure that all staff had fingerprints as required prior to beginning employment.

Evidence:
1. None of the staff records contained completed fingerprint checks.
2. All staff were observed on duty during the inspection.

Plan of Correction: 1. Information for fingerprints was given to the provider. All staff will be fingerprinted and results will be forwarded to the department and placed in staff records.
2. In the future, all staff will be fingerprinted prior to beginning employment.
