Comments:
An unannounced monitoring inspection was conducted with center owner on November 26, 2019 to determine compliance with the licensing standards. There was one infant present with one staff and eight children ages 2.5 years to 4 years present with a second staff. The children were having rest time. Two staff records were reviewed - please see Supplemental Summary for missing items. Lunches are currently being catered by Vinny's. There are reportedly no medications on site.

Based on review of two staff records, the facility failed to have required information for each staff person. Evidence: the staff file for Staff 1 was not available for review on 11/26/2019. The staff file for Staff 2 did not have documentation of two references as to character and reputation as well as competency prior to employment

Description:
Based on review of two staff records, the facility failed to have documentation of a completed fingerprint check on file for one staff prior to employment. Evidence: Staff 1 (date of hire 11/25/2019) did not have documentation of a completed fingerprint check.
Plan of Correction:
Center owner stated the fingerprint check was accidentally sent to another center because the wrong Field print code was used.
