An announced initial inspection was conducted between the hours of 9:00am and 11:00am. A tour of the facility was conducted with the owner. Policies and records were also reviewed.
Based on observation the center did not ensure all areas of the center, inside and outside were maintained in a safe condition. Evidence: 1. There were 2 slats that measured over 4.5" on both sides of the stairs leading to the playground. 2. There was a large wooden giraffe that had an area of splintered wood and peeling paint.
Plan of Correction:
The following corrective action was provided by the owner during the exit interview: Lattice will be placed along the stair slats to ensure there are no gaps. The wooden giraffe will be sanded and a clear paint gloss applied.

Description:
Based on observation the center did not ensure all required non-medical emergency supplies were available. Evidence: The center had an emergency radio, however did not have extra batteries.
Plan of Correction:
The following corrective action was provided during the exit interview: The owner will purchase extra batteries to be kept at the center.
