Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable). A monitoring inspection was initiated on 08/09/2021 and concluded on 08/19/2021. The owner was contacted by telephone to initiate the inspection. There were 46 children present and 7 staff. The inspector emailed the owner a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on observation, the center did not ensure all areas of the center were maintained in a safe condition. The classroom inside the church building had 2 tiles that had cracked and missing pieces.
Plan of Correction:
The following corrective action was provided by the owner during the exit interview: The replacement tiles have already been purchased. They will be installed over the weekend.

Description:
Based on observation the center did not ensure there was at least 12 inches of space between occupied cots. Evidence: During nap, two children were observed sleeping on cots that were less than 12 inches apart from eachother.
Plan of Correction:
The following corrective action was provided by the owner during the exit interview: Owner will ensure staff keep children resting on cots at least 12 inches apart during nap.
