Comments:
An unannounced non-mandated inspection took place on November 15, 2019, beginning at approximately 2:30 PM and ending at approximately 4:30 PM. All staff records and five children?s records were reviewed during the inspection. There were seven children in care and two staff present. There were no medications onsite during this inspection. All areas that are used for care were observed. The inspector looked at first aid and emergency supplies, drill documentation, and required health and fire inspections. The children and staff were observed during the following activities: coloring, a crafts activity, snack time, gym activities and engagement. There were discussions about the children's records and future inspections. The violations were discussed at the conclusion of this inspection. Based on record review and discussion with staff, the center failed to ensure that the separate record for each child enrolled contained all the elements as required by the standards.
Evidence:
1. The record for Child 5 contained a written agreement between the parent and center that was dated 09/23/2019 and the child's documented first day of attendance was 09/19/2019. Staff 2 verified this information. 2. The record for Child 5 did not contain a physical examination by or under the direction of a physician. The child's documented start date was 09/16/2019 and Staff 2 verified that there was no other documentation of a physical examination.
Plan of Correction:
Center will make sure that all agreements are signed prior to the start date. Center will make sure that all official documents are correct and uploaded prior to the child's start date.
