Comments:
On 11/20/2023 an unannounced inspection was conducted in response to a self-reported incident received on 11/09/2023. Interviews and an inspection were conducted regarding the incident. The information gathered during the inspection resulted in a violation. Areas of non-compliance are identified on this Violation Notice. Based on record review and interview, the center failed to ensure a program lead was present and met one of the program lead qualifications.
Evidence:
1. The director stated staff 3 was the lead in the toddler classroom. 2. The record for staff 3 shows a start date of 11/23/20 and a high school diploma. There was no further training, certification or higher education.
Plan of Correction:
Staff 3 was given training to bring her up to date for training. Completed playground safety training and 100 days KC training.
