A Subsidy Health and Safety annual inspection was conducted on July 14, 2022 to review supplemental health and safety requirements. A Religiously Exempt monitoring inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements.
Only subsidy requirements that are not covered in the Religious Exempt Code were reviewed during this supplemental inspection.
Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Based on inspection, the Center failed to ensure staff and children records are maintained and made accessible to the department representative.
EVIDENCE: Staff and children records were unavailable to the Licensing Inspector during the inspection.

Center Director will ensure staff members have access to the children and staff records.
