Comments:
An unannounced monitoring inspection was conducted with the director December 15, 2022 beginning at 3:52 PM and ending at 5:39 PM. There were 41 children, ages 16 months through 6 years old, in the direct care of 9 staff during the inspection. The children were observed playing with age appropriate toys and games and singing songs. Five children?s records were reviewed. Five staff records were reviewed and in compliance. There were no medications on site to be reviewed. The outside play area was inspected. The most recent emergency evacuation drill was documented as 11/29/2022. The most recent shelter in place drill was documented as 5/25/2022. The most recent health inspection was dated 9/28/2022. The most recent fire inspection was dated 2/01/2022.

Based on a review of documentation, the facility did not ensure that all the children's records contained the required information. Evidence: Child A and child B?s records did not have documentation of parent confirmation that the required information in the children?s records were up to date for 2022.
