Comments:
A monitoring inspection was initiated and concluded on 6/26/2023. There were 3 children present with the provider. The point total was 8. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 2 Provider / household member records were reviewed. The inspection started at 12:10 pm and concluded at 1:35 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-850-90-C
Description: Based on a review of records and interview, the provider sis not ensure that each enrolled child has a record with all required documentation to include the name of the parents' hospitalization plan and number or medical assistance plan, if applicable and the results of a health examination.
Evidence:
Four children's records were reviewed.
The records for child #1 and child #3 did not contain the number for the parent's plan and number or medical assistance plan listed on the form.
The records for child #1 and child #3 did not contain the results of a health examination.
The provider confirmed that this information was not present in the records during the inspection.

Plan of Correction: The provider will ask the parents to supply the missing documentation by the end of the week and will review records at the time of enrollment to ensure a complete record is obtained for each newly enrolled child.

Standard #: 8VAC20-850-110-C
Description: Based on a review of records, the provider did not meet all criteria listed on the Health and Safety checklist at the time of inspection.
Evidence:
The provider did not ensure steps to safeguard the home from all open and obvious household hazards.
The hot water temperature at the handwashing sink where children wash their hands measured 136 degrees Fahrenheit with a digital thermometer. Children should not have access to water over 120 degrees Fahrenheit due to burn risk.

Plan of Correction: The provider will continue to accompany the young children in care to the restroom and adjust the water temperature before children wash their hands.
The hot water heater will be adjusted to bring the temperature to no more than 120 degrees Fahrenheit and rechecked to ensure it stays at a safe temperature.
