Comments:
A complaint inspection was initiated on 4/20/23 and concluded on 4/20/23. There were 74 children present with 11 staff supervising. The inspector reviewed compliance in the areas of physical plant, and staffing and supervision. The inspection started at 11:00am and concluded at 11:45am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on observation and discussions with staff the facility failed to ensure that the space used by the children was safe and operable as required.
Evidence: During observation of the classrooms, on 4/20/23, the Licensing Inspector found that the exit door of the infant room would not open. This door is an egress door to the outside and used for emergency exit for fire. The staff were using another door in another classroom to get outside to the play area.
Plan of Correction:
Maintenance will immediately look at the door and fix it so it will be able to be opened and used as required.

Based on staff information and observations the facility failed to ensure that ratios were met as required.
Evidence: The licensing inspector observed, on 4/20/23 at 11am, that there were 34 children aged 2years-4years having water time outside. There were only 3 staff present; the required ratio for this mixed age grouping was 1 staff to 8 children. Staff and documentation verified that the group became out of ratio at 9am. The ratio for this group was not met from 9am-11:45am, until the group separated.
Plan of Correction:
2 staff have gone to get fingerprinted today. One can start May 22 and one has been hired.

Based on documentation and staff statements the facility failed to ensure that ratios were met as required.
Evidence: On 4/20/23 the licensing inspector verified through staff statements, documentation, and observation that :
1. The ratio for the 3-4 year old group was not met from 7:45am-8:15am as there was only 1 staff with 25 children. The required ratio was 1 staff to 10 children.
2. On 4/19/23 the 4-5 year old group was also out of ratio from 9am-3pm with 1 staff and 11 children. The required ratio was 1 staff to 10 children.
3. On 4/20/23 the 3-4 year old group was out of ratio from 11:45am-3:30pm with 2 staff and 26 children. The required ratio was 1 staff to 10 children.
Plan of Correction:
2 staff have gone to get fingerprinted today. One can start May 22 and one has been hired.
