Comments:
An unannounced monitoring inspection was conducted today from approximately 12pm-1pm. There were 5 children (12 points) in the direct care and supervision of 1 caregiver. A sample of 3 children?s records,1 caregiver's record were reviewed. The children were observed napping and drawing. The provider is contemplating adding an assistant. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Based on record review, the children's records did not contain required information.
Evidence: 1. Child A was missing the employer and and employer phone number of both parents. 2. Child B was missing the fathers employer phone number and the address of an emergency contact. 3. Child C was missing proof of identification.
Plan of Correction:
Provider said I will get the information today.
