Comments:
An unannounced monitor inspection was conducted with the provider beginning at 11:11 AM and ending at 12:23 PM. There were 5 children in care with one household child, ages 15 months, 19 month, 2, 2, 2, and 4 years old for a total of 15 points. The children were observed eating lunch and preparing for nap time. Three children's records were observed and in compliance. Two household records were reviewed and in compliance. There were no medications on site to be reviewed. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. The most recent emergency practice drill was documented as 12/14/2022. The more recent shelter-in-place drill was documented as 10/10/2022.

Based on a review of documentation, the provider did not ensure that each child was recorded on the daily written attendance. Evidence: There were 2 children signed in on the written attendance and 5 children physically present.

The written attendance sheet was updated during the inspection. The provider stated that she would ensure each child was signed in upon arrival.

Based on a review of documentation, the provider did not ensure that all of the children's records contained the required information. Evidence: Child A's record was missing one emergency contact. Child B's record was missing two emergency contacts.

Description:
Based on observation, the provider did not ensure that The diapering surface was separate from surfaces used for children's activities. Evidence: The provider was observed changing a child's diaper on a diapering pad placed on a table used for children's activities.
Plan of Correction:
The provider stated that she would no longer change diapers on top of the table utilized for children's activities moving forward.
