Technical Assistance:
The Virginia Department of Education (VDOE) assumed oversight of child care licensing and monitoring on July 1, 2021. All associated regulations and codes have transferred over from the Virginia Department of Social Services. The violations listed below reflect the new VDOE codes and standards. A crosswalk document has been provided for the new regulation numbers. Discussed with provider: child record requirements. Model forms were sent to the provider electronically.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program. A monitoring inspection was initiated on 1/26/22 and concluded on 1/26/22. The provider was contacted by telephone and a virtual inspection was conducted. There were 2 children present, (both 2 years old), with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child records and 1 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on a review of records, one child record does not contain documentation of a completed physical past 30 days of enrollment. Evidence: Child A does not have a physical one file.
Plan of Correction:
The parent was contacted and asked to provide the physical for the file. The doctor's office was contacted today.

Description:
Based on a review of records, one child record does not contain documentation of an updated record. Evidence: Child #1's record does not contain an updated enrollment form. The previously viewed form was not available. The form reviewed did not contain all required information as it was another agency's form.

The enrollment form was sent home with the child to be completed. It will be returned tomorrow.

Description:
Based on a review of records, one child records does not contain written acknowledgment of all required policies and procedures. Evidence: There is no acknowledgment of the Liability Insurance Declaration (Child A).

The form was sent home with the child for signature and will be returned tomorrow.

Description:
Based on a review of provider records, there was no subsequent TB screening obtained within the allowable two year time frame. Evidence: The TB results on file are dated 1/181/19 (expired).

I will try to get the TB test done this weekend.
