Technical Assistance:
Reminder: All staff are required to obtain 16 hours of training annually.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated and concluded on July 1st, 2020. The Director was contacted by telephone to initiate the inspection. There were 6 children present and 4 staff. The Inspector reviewed 4 children's records and 4 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of children's records, the center failed to obtain documentation of the child's immunizations by the first day of attendance.

Evidence:
Child #2's first date of attendance was 5/13/2020; however the documentation of immunizations was not dated until 6/1/2020.

Plan of Correction: I will remember to require immunization from new students enrolled on the first day of attendance.

Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records, the center failed to ensure that each child shall have a physical examiniation by or under the direction of a physician within one month after the first day of attendance.

Evidence:
Child #2's first date of attendance was 5/13/2020; however, documentation of a physical was dated 6/25/2020.

Plan of Correction: I will remember to require physicals by the 30th day of attendance as new students enroll.
