Comments:
An unannounced monitoring inspection was conducted today from 12:00pm-1:45pm. There were 17 children (3-4 years old) directly supervised by 2 staff. The physical plant, 2 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed eating lunch and watching a movie. There was an adequate supply of materials for the children. The program was clean and organized. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Repeat Violation Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child #3's record contains documentation of immunizations that were last updated when the child was 14 months old.

Front office will contact the parents for updated immunizations.

Evidence: 1. Child #3's record was missing the father's place of employment. 2. Child #5's record was missing the father's place of employment and the address for both emergency contacts.
