Comments:
A monitoring inspection was conducted 3/22/24 from 9:32am-11:20am. 8 children, supervised by 2 staff in one classroom and 2 children with staff in a second classroom. Children were engaged in writing letters and numbers. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care, emergencies and nutrition. A total of 5 child records and 1staff record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the center did not obtain documentation of a negative tuberculosis screening for staff. Evidence: Staff #1 record did not contain results of TB screening.

REPEAT Based on record review children's records did not contain all required information. Evidence: 1. Children #1, #2, #3, #4 and #5 records did not have address for two emergency contacts. 2. None of the 5 children files had documentation of viewing proof of child's age and identity.

Front office staff will contact parents to obtain required information of children currently enrolled and will ensure documentation is complete at enrollment for new students.
