Comments:
The licensing inspector conducted an investigation in response to a self-reported incident that was received by the department on February 9, 2024 regarding allegations of inadequate supervision. The inspection was initiated and completed on February 9, 2024. The inspector was on site from 1:25 pm to 3:45 pm. Interviews with staff and child were completed, observations were made in the classroom, and a video recording was viewed.

The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the self reported incident is determined to be valid.

Standard #: 8VAC20-780-340-F
Description: Based on staff interviews and observation of the center's video recording, children under the age of 10 years of age were not always within actual sight and sound supervision of staff.

Evidence:
1. A video recording on Thursday February 8, 2024 of the 4?s classroom during naptime shows Staff #1 sitting at a table in the middle of the room, and most children on their cots. At the 1310 mark on the video, Child #1 was observed walking to Child #2's cot by the classroom door, out of view of the camera. At the 1319-mark, Child #1 was seen walking around the classroom. At 1321, Child #1 returned to Child #2's cot until seen walking around the room at 1334. At 1335, Child #1 went back to Child #2's cot. At 1338, Staff #2 entered the room and sat in the corner at a desk. Staff #1 left the classroom at 1342. Child #1 remained at Child #2's cot until 1406 when Staff #2 turned on the lights and the rest of the children started getting up from naptime.


2. Staff interviews confirmed that while Staff #1 sat at the table in the middle of the room and while Staff #2 sat at the desk in the corner of the room, they could only see the corner of Child #2's cot. The staff could not see Child #1 and Child #2 during those times.

Plan of Correction: The room has been arranged so that all cots are within sight vision of staff during naptime.
