Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 4/26/2021 and concluded on 5/3/2021. The provider was contacted by email to initiate the inspection. There were 4 children present with the provider. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and the provider's record submitted by the facility to ensure documentation was complete. The information gathered during the inspection determined there were violations which are detailed on the Violation Notice. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review it was determined the provider did not ensure that parents reviewed and documented the emergency contact information listed in their child's record annually. Evidence: 1. The record for child #1 and child #2 did not contain documentation of annual review. 2. The provider confirmed that the records did not contain documentation of annual review.

Provider will have parents review and sign/date the records.

Description:
Based on record review it was determined the provider did not obtain documentation of additional immunizations once every six months for children under the age of two. Evidence: 1. The record for child #2 did not contain updated immunization records until they were two. The last immunization record was dated 7/16/19 at which time the child was 18 months old. 2. The provider confirmed that there was no update immunization documentation for child #2.
Plan of Correction:
Provider will request updated immunization records for child #2.
