Inspector spoke with provider about out buildings during this inspection.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and video tour.
A monitoring inspection was initiated on 11/1/2021 and concluded on 11/8/2021. The provider was contacted by email to initiate the inspection. There were 5 children present with the provider and assistant provider. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 provider/household member records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review it was determined the center did not ensure that all children's records contained all of the required components. Evidence: The record for child #2 did not contain 1 of the 2 required emergency contacts.
Plan of Correction:
Provider will request an additional emergency contact from the parent(s).
