Comments:
An unannounced monitoring inspection was conducted on 07/19/2023. At the time of entrance, 10 children were in care with the assistant provider. The record sample size consisted of 5 children's records and the provider's record and the assistant's record. The children in care were observed engaging with the provider, participating in nap. There have been no injuries since the last inspection and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.
Based on observation it was determined the provider did not ensure that children are not allowed to drink while laying down. Evidence: 1. Child #4 (5 months old) was reclined in an infant rocker seat at nap time with a bottle.

Description:
Based on record review it was determined the provider did not ensure that caregiver records contained all the required information. Evidence: 1. The record for assistant #2 (hired 12/22/22) did not contain documentation of the orientation as required by 8VAC20-800-230. 2. The record for assistant #1 (hired 4/21/23 did not contain documentation of the time of arrivals and departures as a substitute provider as required by 8VAC20-800-140 D.

Description:
Based on record review and interview it was determined the center did not have a written record of children in attendance each day. Evidence: 1. The attendance sheet dated for 7/19/23 contained 8 children's names. 2. At time of inspector's arrival 10 children were in care, 8 of the 10 were listed on the sign in sheet. 3. Staff confirmed that there were 10 children in care and only 8 were on the sign in sheet during this inspection.

Based on observation and interview it was determined the provider did not ensure that each caregiver did not exceed 16 points. Evidence: 1. There were 10 children in care totaling 26 points with the assistant provider at the time of inspectors arrival at 2:15pm. 2. The substitute provider returned at approximately 2:30pm. 3. The assistant provider confirmed they were the only provider at the time.
