Comments:
An unannounced code compliance inspection was initiated on September 25 and concluded on September 26, 2023. There were 56 children in care ages two months through five years old with 10 staff. Children were observed in teacher and self-directed activities. Staff background checks and health reports as well as children's immunizations were reviewed.
Based on record review the center failed to ensure each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Evidence: 1 - The following staff records have health reports which are more than one year old and therefore past due for update: a. The most recent health report in the record of Staff #1 is dated 2019. b. The most recent health report in the record of Staff #2 is dated December 2020. 2 - The following staff records do not contain any health report documentation: Staff #3 and #4
