Comments:
An unannounced complaint investigation was initiated on 1/31/2024 and concluded on 3/28/2024 in response to a serious injury report received on 1/31/2024 and a complaint received by the licensing office on 02/15/2024 regarding supervision. Interviews were conducted relating to the allegations on 02/06/2024, 02/20/2024, and 03/04/2024. The information gathered during the investigation does support the allegation, so the complaint is determined to be "valid". Based on interview and record review it was determined the center staff did not ensure the care and protection of the children in their care. Evidence: 1. A serious injury report submitted to the Office of Child Care Health and Safety on 1/31/24 by the center stated child #1 fell off a tree stump on the playground and broke their collarbone. 2. On 2/14/24 child #2 received a cut under their eye from a PVC pipe that another child was playing with while on the playground. 3. Staff interviewed confirmed these injuries occurred while child #1 and child #2 were in care.

REPEAT VIOLATION Based on interview and record review it was determined the center did not ensure that all children under the age of 10 were always within actual sight and sound supervision. Evidence: 1. On 1/30/24 child #1 fell off a stump and broke their collarbone, staff reports and interview confirmed staff did not see child #1 climbing on the stump or falling off of it. 2. On 2/14/24 child #2 got hit with a piece of PVC pipe by a peer resulting in a cut under their eye, staff interviewed stated that they did not see the incident that led to the injury.
