An unannounced renewal inspection was conducted May 31 2022 between 10:00 and 11:00am. There were 2 children and one provider child in the direct care of the provider. 2 child records, 2 household records, and the physical plant of the home was inspected and reviewed. Based on record review, 1 of 2 children's records reviewed did not contain written agreements signed by the parents by the end of the first day of care.

Based on household record review,the provider did not ensure that each household member had a Current Report of Tuberculosis Screening every two years from the date of the first screening.
Household member #1 and #2 Tuberculosis Screening records are dated October 28, 2019.
