Comments:
A complaint investigation was initiated on March 10, 2020, regarding an 8-year-old injury on school-age playground and policies for emergencies wer not follow. Interviews were conducted, and documentation and video-camera footage were reviewed. There was a preponderance of evidence to support the allegation regarding the child?s injury and written record of child?s injury were not followed.
The complaint related violations and one non-related violation are identified in the Violation Notice.

Based on records review and staff statement, the center did not notify the parent by the end of the day of any known minor injuries. Evidence: Child #1 parent was not notified of child's incident injury by the end of the day.
