Comments:
An unannounced renewal inspection was conducted on 01/11/2024 from 9:40am to 10:40am. There were 2 children ages 14 months to age 2 years old (6 points) with the Provider. Children were observed participating in free play and eating snack. The Licensing Inspector and the Provider discussed the Health & Safety checklist. Noncompliance with the checklist was observed and was discussed with the Provider. Violations can be reviewed on the Violation Notice. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 1/18/23.
Based on review of 3 Household member records, the Provider did not obtain the results of a central registry check or Criminal History check from all states in which Household members have resided within the last five years by the end of the 30th day of residence
