Comments:
A renewal inspection was conducted on 8/31/2022. There were 5 children present, totaling 11points, with the provider supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 caregiver/household records were reviewed.
Information gathered during the inspection determined non-compliance with an applicable standard and a violation was documented on the violation notice issued to the program.
Based on review of children's records and provider interview, the provider has not ensured that each child's record includes the names, addresses and telephone numbers for two persons to be contacted in an emergency when a parent cannot be reached. Evidence: 1. The records provided for child 2, 3, 4, and 5 did not include the street addresses for one of the two listed emergency contact persons. 2. The provider verified that records listed above did not include the street addresses for one of the two emergency contact persons required.
Plan of Correction:
The provider responded with the following: All parents were contacted and provided the street addresses for the emergency contacts by the end of the day. In the future, all parents will be required to provide the names, addresses and telephone numbers for two emergency contact persons.
