Technical Assistance:
*Please ensure that when the crew is done with last minute touches in the facility that their work equipment, materials and debris is all cleared and removed prior to children attending the facility. *Ensure that documentation of age verification is always kept in staff records for verification during licensing inspections. *Ensure that all keys to locked cabinets are kept inaccessible to children at all times. Please review standards 280-B, 510-K, etc. *Ensure that clear documentation of staff qualifications are kept in their files: transcripts, copies, etc.
Comments:
An Announced Initial Inspection was conducted today from 10:15am to 1:30pm. There were no children in care at the time of the visit, and staff were training and preparing classrooms. The physical plant, 12 staff records, 1 child?s records, emergency supplies, and policies were inspected. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. The center was going through finishing touches and was well organized. Based on review, the facility did not ensure that the obtained documentation of qualified program leaders. Evidence: Reviewed 12 staff records and found that Staff 4 and Staff 6 did not have documentation of qualifications to be a program leader.
Plan of Correction:
Both staff have requested transcripts. If not received then status will change.
