Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on February 18, 2022. The provider was contacted by telephone and a virtual inspection was conducted. There were no children present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of zero child records, one household member's record, and one staff record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the provider did not review the emergency plan at least annually and update the plan as needed. Evidence: 1. The provider did not have documentation of an annual review of the Emergency Preparedness and Response Plan. The Emergency Plan was last reviewed on 05/01/2020.

Description:
Based on record review and interview, the provider did not obtain subsequent tuberculosis screenings for each caregiver and adult household member. Evidence: 1. Staff #1's record contained an expired tuberculosis screening. The screening was dated 01/10/2020. 2. Household member #1's record contained an expired tuberculosis screening. The screening was dated 01/10/2020.
Plan of Correction:
The provider and household member will go today to get a new tuberculosis screening.
