The licensing inspector initiated an unannounced inspection on 03/28/2023 and concluded on 03/28/2023 in response to a complaint received by the department on 03/17/2023 for allegations regarding supervision and physical plant. Interviews and observations were conducted relating to the allegations. The information gathered during the investigation does not support the allegation, so the complaint is determined to be "not valid." There was a violation found during the complaint investigation. Non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. If you have any questions, contact your licensing inspector at (540) 848-4123.
Based on record review and interview, the center failed to obtain all required information for a child's record.
Evidence:
1. The record for child 1 was reviewed. The record did not contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. 2. The director verified they only had the parents contact information.
Plan of Correction:
The emergency contact information for child 1 will be obtained and documented in the child's record. In the future the administration will ensure all required documentation for children's records are complete prior to first date of care.
