Model forms for action plans and a list to maintain in the kitchen can be found on the Licensing website.
Subsidy annual update training should be completed once a year.
Comments:
A subsidy health and safety (SHSI) inspection was conducted on 08/09/2023.. There were 8 children present with 2 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on provider statement and record review, a child with a listed food allergy does not have a care plan. Evidence: Child #1 is missing a list of instructions from the parent/physician that documents which food the child is allergic to and the steps to take in the event of a suspected allergic reaction.
