Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on May 3, 2021 and concluded on May 3, 2021. The provider was contacted by telephone to initiate the inspection. There were nine children present and two caregivers. The inspector emailed the /provider a list of items required to complete the inspection. The Inspector reviewed two children?s records and two caregiver/ one household member records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on interview, the provider did not document in writing each annual review and update to the emergency plan.
Evidence:
The provider stated she reviewed the plan however she had not documented the annual review. The most recent review was documented on 03/05/2020.

Per the provider: I am reviewing the plan today and documenting the review in writing.

Description:
Based on review of two children's records, the provider did not obtain all required information for each child's record.
Evidence:
The record of child #1, enrolled 12/08/2020, did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately. The record also did not contain the parents signed acknowledgement of the receipt of the policies for the administration of medications. 2. The record of child #2, enrolled 03/09/2020, did not contain the parents signed acknowledgement of the receipt of the policies for the administration of medications.
Plan of Correction:
Per the provider: I will have the parent complete the missing information at pick up.
