Comments:
An unannounced monitoring inspection was conducted on 6/28/2024. The inspector was on site from approximately 10:40 am-1:40 pm. There were 7 children in care, ranging in age from 1 year to 4 years with two caregivers supervising. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, care of children, emergencies, caregiver training, preventing the spread of disease and nutrition. A total of 7 children?s records and 2 caregiver records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it within 5 business days. Please specify how the deficient practice will be or has been corrected.
Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on review of two caregiver?s records and interview, the assistant caregiver did not obtain a minimum of 16 clock hours of training annually in areas relevant to the job responsibilities.
Evidence: The assistant had documentation of 3 hours of training in 2023. The provider and the assistant acknowledged that the assistant had not followed through with the annual training requirements.
