Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on June 8, 2020 and concluded on June 9, 2020. The provider was contacted by telephone to initiate the inspection. There were nine children present and three staff. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed two children?s records and three staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.
Based on review, facility did not ensure 1 of 2 children's records has documentation of being adequately immunized prior to beginning attendance.
Evidence: The record for child #1 (start date June 2, 2020) did not have an immunization record.
Plan of Correction:
I will make sure that in the future all children have their immunization record before they start attending. On 6/8/20 was this child last day.

Description:
Based on review, facility did not ensure 1 of 2 children's records has documentation of required information.
Evidence: The record for child #1 (start date June 2, 2020) did not have the address for the second emergency contact and allergy information.
Plan of Correction:
I will make sure that in the future all children have the emergency contact address and allergy information. On 6/8/20 it was this child last day.

Description:
Based on review, facility did not ensure 1 of 3 staff records has a completed central registry check within 30 days of beginning employment.
Evidence: The record for staff #1 (start date April 22, 2020) did not have a completed central registry check.
Plan of Correction:
I have sent the central registry background check paper and we are waiting for the result.
