Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was conducted on December 8, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were two children present with one caregiver supervising. A total of two child records and one caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on review, provider did not ensure 2 of 2 children's records have documentation of required information.
Evidence: The following was found during record review: 1. The record for child #1 (start date December 6, 2021) did not have emergency contact address and physician information (name, address and phone number). 2. The record for child #2 (start date November 4, 2020) did not have father's employment information (place, address and phone number), emergency contact address and physician information (name, address and phone number).
Plan of Correction:
Information will be requested from parents and added to child's record.

Evidence: The record for child #2 (start date November 4, 2020) did not have a physical examination within 30 days of enrollment.
