Comments:
A monitoring inspection was conducted on November 13, 2023 at the provider's new location to review voluntary registration requirements. There were two children present, ages three years, with one caregiver. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and emergency supplies. A total of two children?s records and two caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-850-30-C-2-b
Description: Based on review, provider did not ensure caregiver has an additional negative tuberculosis (TB) screening/test every two years.

Evidence: The provider record had documentation of a negative TB screening/test dated October 19, 2021 which is more than two years old.

Plan of Correction: Provider will have TB screening completed on Saturday.

Standard #: 8VAC20-850-110-C
Description: Based on observation, provider did not ensure the criteria on the health and safety checklist is met at the time of the monitoring visit.

Evidence: The following was found during the monitoring inspection:
1. Two cords were observed coming from the flat screen television mounted on the wall down to the electrical outlets. The cords were within reach of children.
2. There was not a protective barrier at the stairway accessible to children. The safety gate at the bottom of the stairs was not secured to the wall on one side. Provider stated the connecting latch on the wall had broken off.

Plan of Correction: TV cords will be secured and out of reach of children.
The wall latch will be replaced for the safety gate.
