Comments:
A monitoring inspection was initiated on 09/07/2021and concluded on 09/10/2021. The provider or in-charge person was contacted by telephone to initiate the inspection. There were six children present with the provider. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed one child?s records, along with any requested program records submitted by the facility to determine if required documentation was complete.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
Based on a review of one child's file, it was determined that the provider did not ensure that all required information was present in each child's record.
Evidence: The record for child #1 did not include addresses for either of the emergency contact persons.
Plan of Correction:
Provider will obtain addresses and place them in the record.
