Comments:
A monitoring inspection was conducted from 9;30 to 10:30 AM. The provider was present with two children (six points). Children were observed watching television and playing with toys. The sample size was two children's records.
Based on a review of two children's records, it was determined that the provider did not ensure that elements were present in each child's record.
Evidence: !. The record for child #1, who was in care at the time of the inspection, contained only one of the two required emergency contact persons. 2. The record for child #2, who was in care at the time of the inspection, did not contain addresses for the two emergency contact persons. 3. Provider verified that information was missing.
Plan of Correction:
Provider will obtain required information from parents. Additionally, all children's records will be reviewed to ensure that they are complete.

Description:
Based on observation, it was determined that the vendor did not ensure that all areas of the home, inside and outside were maintained in a safe manner.
Evidence: There were nails sticking out on the porch that the children pass by on their way to the outdoor play area

Provider's husband will pound the nails back in this evening.

Description:
Based on observation of the areas used by the children, it was determined that the provider did not ensure that children were protected from choking hazards.
Evidence: There was an open package of small batteries on a table in the living room.
Plan of Correction:
The provider will ensure that all choking hazards are placed in areas where they will not be accessible to the children in care.

Description:
Based on observation at the home, it was determined that the provider did not ensure that potentially poisonous substances are kept in areas inaccessible to the children.
Evidence: There were multiple potentially poisonous substances found throughout spaces used by children including, but not limited to: clorox wipes on the back of the toilet, Goody hangover pills on a table in the living room and hair gel on a table in the living room.
Plan of Correction:
Provider will remove all potentially poisonous substances to locked locations.
