Comments:
An unannounced renewal inspection was conducted from 1:00 - 2:30pm with the provider and her assistants. There were 12 children in care, ranging from 13 months to four years of age, for a total of 29 points. Children were observed napping, waking, and playing with age-appropriate toys. Five child records, five caregiver/household member records, the emergency drill log, daily attendance, first aid kit, and menu were reviewed. The provider is certified in Medication Administration Training (MAT) as of 1/18/2023 and can administer medication to children in care. If you have further questions about this inspection please contact Donna Liberman at 540-359-5244 or Donna.Liberman@doe.virginia.gov.
Based on a review of five child records, the provider failed to obtain documentation that each child has received a physical examination prior to the child's first attendance or within one month after attendance. Evidence: the record for child #5 (date of first attendance: 10/18/21) did not have documentation of a physical examination.
Plan of Correction:
Will obtain a copy of the physical from the parents.
Standard #:

Description:

Plan of Correction:
The household member will obtain an updated TB.
