Comments:
A renewal inspection was initiated and concluded on June 27, 2022 to determine compliance with the requirements for Voluntary Registration. The licensing inspector was on site from approximately 11:30am until 1:10pm. The provider assisted the licensing inspector during the inspection. There were two children in care and one caregiver present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and medication. The inspector reviewed one child and one provider?s record.

Information gathered during the inspection determines non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-850-90-B
Description: Based on documentation review and interview, the provider did not maintain a signed statement from each parent, affirming receipt of the information to parents statement.

Evidence:
Child #1 was being cared for the provider during the inspection. The provider stated she did not have a signed statement from the parents of Child #1, affirming receipt of the information to parents statements.

Plan of Correction: I will follow up with the parents and make sure they complete the required paperwork.

Standard #: 8VAC20-850-90-C
Description: Based on documentation review and interview, the provider did not ensure a record was maintained for one child enrolled in care.
Evidence:
Child #1 was being cared for the provider during the inspection. The provider stated the parents for Child #1 has all the paperwork to complete and they had not turned it in. The children's records are required to contain the name, address and telephone number of each parent's place of employment and his or her work hours; the name, address and telephone number of one or more persons designated by the parent or parents to be called in case of emergency when a parent cannot be reached during the hours the child is in care; any known or suspected allergies and any chronic or recurrent diseases or disabilities; the child's allergies to medication or drugs, if applicable, and directions for providing medicines to the child; the name of the parent's hospitalization plan and number or medical assistance plan, if applicable; the parent's signed authorization for the child's emergency medical treatment and
written consent for giving of medications to the child; the child's date of enrollment; names of persons authorized to visit or call for the child, as well as those who are not to visit or call for the child; any written agreement made between the family day provider and the natural parent, guardian, or other responsible person for each child in care. The agreement may cover hours of care per day, week, or month; cost of care per day, week, or month; frequency and amount of payment per day, week, or month; and any special services to be provided by either party to the agreement.

Plan of Correction: I will follow up with the parents and make sure they complete the required paperwork.

Standard #: 8VAC20-850-110-C
Description: Based on observations, documentation review and interview, the provider did not meet all the criteria on the health and safety checklist at the time of the inspection.
Evidence:
During the June 27, 2022 the following was observed:
1. The provider stated she did not have a record for Child #1. Children?s records are required to have the following: documentation of immunizations prior to or on the date of admission; a medical release form from each family to permit emergency care; the names and phone numbers of one or more persons besides the family who may be contacted in case of an emergency; documentation of previous child day care programs and schools the child has attended and documentation that proof of identity was reviewed and the date of review.
2. The record for Child #2 (start date 01/13/2022) did not contain documentation of a physical exam.
3. A contained of nighttime cold and flu over the counter medication sitting on table located by the couch. Two children were present and being cared for during the inspection.
4. Two cats were observed living inside the home. The provider stated the cats have not received rabies shots.

Plan of Correction: 1. I will have the parents of the children turn in the required paperwork.
2. I will call local vet and rabies clinics to get the cats rabies shots.
3. The medication was moved during the inspection.
