Comments:
An unannounced renewal inspection was completed on 11/14/2022. There were 101 children present with 20 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 9 staff records were reviewed. The inspection started at 1:00pm and concluded at 4:30pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review, the center failed to get an out of state central registry check on an employee who had lived out of stare in the last 5 years. Evidence: 1. Staff #1 was hired 09/07/2022 and had indicated on the sworn statement they had lived out of state in the last 5 years, and the out of state central registry check had not been completed.

The out of state central registry check will be submitted.

Description:
Based on record review, the center failed to have employees submit documentation of a negative tuberculosis screening at the time of employment and prior to coming to contact with children. Evidence: 1. Staff #1 was hired 09/07/22 and the TB screening was completed 11/10/22. Staff #3 was hired 09/14/22 and the TB screening was completed 10/24/22.

TB screening documentation will be submitted at the time of employment and prior to coming into contact with children.

Based on observation, the center had a trampoline. Evidence: 1. In the gym there was an individual sized trampoline.
