Comments:
An unannounced renewal inspection was conducted on 8/30/22 from 9:40am until 11:00am. At the time of entrance, there was 1 child in care for at total of 3 points with the provider. One child record and one provider record was reviewed. The child was observed engaging in free play with age appropriate learning toys. Diaper changing procedures were also observed. First aid and emergency supplies, documentation of emergency drills and the evacuation plan were reviewed. The provider stated she does not provide transportation and does not administer medication to children in care at this time. Information gathered during the inspection determined non-compliance with applicable standards or law. Violations are listed on the violation notice and were discussed with the provider during the exit interview.

Violations:
Standard #: 8VAC20-850-90-C
Description: Based on record review and interview, the provider did not ensure that children's records contain all the required information.

Evidence:
1. The record for child 1 (date of enrollment: 2/13/22) does not contain the name and address of each parent's place of employment and does not contain the address of the person designated by the parent to be called in case of emergency when a parent cannot be reached during the hours the child is in care.
2. The provider confirmed that the record for child 1 is lacking the parent's work name and address and the address for the emergency contact.

Plan of Correction: The provider responded with the following: The parent of child 1 will be contacted to obtain the missing items.

Standard #: 8VAC20-850-110-C
Description: Based on record review and interview, the provider did not ensure that each child's record contains documentation that proof of identity was reviewed and the date of review and that monthly practice evacuations drills are documented.

Evidence:
1. The record for child 1 (date of enrollment: 2/13/22) does not contain documentation that proof of identity was viewed.
2. There is no documentation that monthly practice evacuations drills have been completed.
3. The provider confirmed that the record for child 1 is lacking documentation of proof of viewing the child's birth certificate and that practice evacuation drills were not documented each month.

Plan of Correction: The provider responded with the following: 1. The parent of child 1 was contacted during the inspection and a copy of the birth certificate was obtained and placed in the child's record. 2. Evacuation drills are practiced monthly; however, they were not documented in the log. Going forward, the provider will document all required practice evacuation drills in the drill log.
