Comments:
A subsidy health and safety inspection (SHSI) was conducted today to review supplemental health and safety requirements not covered in the licensing regulations. A licensing inspection was conducted on the same date, and the two inspections combined cover all SHSI requirements. One area of non-compliance was found in the supplemental subsidy health and safety requirements. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Based on information received, it was determined that the provider has not completed a required annual training. Evidence: The provider reported that she has not completed the Virginia Health + Safety Training in the last 12 months.
Plan of Correction:
The training will be completed in the next 30-days.
