Comments:
A renewal inspection was conducted on 01/11/2024 between the hours of approximately 9:30am and 11:20am. At the time of entrance, there were 4 children present with 2 caregivers supervising. An additional child arrived during the inspection, for a total of 5 children (10 points) with 2 caregivers. Children were observed having positive interactions with staff, eating snack and working with various age-appropriate materials. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming and emergencies. There were no medications on site today. A total of 2 child records 2 staff records and 1 household record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on children?s record review, the provider did not obtain, annually, a signed acknowledgement of emergency contact information review.
Evidence: The record for Child #1 did not have a signed acknowledgment that emergency contact information was reviewed. The most recent information review documented in the record was dated 04/28/22.

I will have the parent review the information and sign the acknowledgment today.

Description:
Based on observation and interview with the provider, food brought from home was not clearly labeled in a way that identifies the owner.
Evidence: On the date of inspection, three containers of food were observed in the family day home?s refrigerator. These containers were not labeled with the name of the child. Upon interview, the provider identified the containers with the owner's name.

Description:
Based on observation, unused electrical outlets did not have child- resistant protective covers.
Evidence: 1) In the bathroom of the care area, there was one unused outlet with no protective cover on the left wall next to the sink.
